New Patients
We want your experiences at the Pediatric Group to be exceptional and doing a few simply things, you can help to ensure that your visit runs smoothly. Make sure that you arrive at least 15 minutes early to your first visit to allow us time to check you in and obtain all of the important information that we will need. Bring you insurance card! In order to accurately submit claims to your insurance company, on your behalf, we require that patients bring their current insurance card to every visit. We will need to know policyholder information as well as demographic information for you and your family.
To save time at check in and in the waiting room, you can complete some of the necessary paperwork in advance. You will need to complete a 2006 Family Information Card and a patient history, based on your child's age, which can be selected from the list below. If your child will be having a well visit, the following section contains additional forms that will need to be completed for the visit.

We know it's not easy to juggle a clipboard and an active toddler. We have made available to our patients many of the forms that we will need for your visit. By completing necessary paperwork in advance, you will save time checking in and in the waiting room, in the comfort of your own home! Click below on the visit type that your child is scheduled for and you will be able to fill out and print forms in advance of your visit. As we make this feature available to our patients, please remember that some paperwork will still have to be provided by our staff and completed at the office.
AD/HD Packet
This comprehensive packet of information includes several forms to be completed by both parents and your child's teachers, in addition to reference materials to educate you on the types of Attention Deficit Hyperactivity Disorder (ADHD) and the problems that accompany this condition. The information you and your child's teacher(s) provide will assist your provider with determing if your child has any clinical or diagnostic findings consisten with ADHD.
Once you have collected the appropriate information and completed the necessary forms, please return them to your child's primary care provider at the office location where your child's medical records are retained. Once the completed forms have been reviewed, your child's provider will contact you to schedule a consulation appointment. CLICK HERE to obtain a complete ADHD packet.
