We know it's not easy to juggle a clipboard and an active child. We have made available to our patients many of the forms that we will need for your visit. By completing necessary paperwork in advance, you will save time checking in and in the waiting room, in the comfort of your own home! As we make this feature available to our patients, please remember that some paperwork will still have to be provided by our staff and completed at the office.
